We are looking for a dedicated, and compassionate Mental Health Therapist who will play a crucial role in providing mental health services within our primary care setting. As a vital member of our healthcare team, the Mental Health Therapist is responsible for offering a range of services, including population-based education, early intervention, and participation in the management of chronic diseases. This role encompasses conducting psychosocial assessments to identify the strengths and challenges faced by members concerning their illness, treatment, and overall life situation.

KEY RESPONSIBILTIES:
- Provide comprehensive mental health care and support, including the diagnosis and treatment of mental health disorders.
- Develop tailored treatment plans based on individual patient needs and circumstances.
- Collaborate within an interdisciplinary team, participating in treatment team meetings to discuss clinical cases and update patient treatment plans.
- Engage in continuous monitoring and coordination of behavioral health services, linking with other treatment providers both within and outside the primary care setting as permitted.
- Maintain detailed treatment and progress reports, documenting in patient medical charts.
- Pennsylvania Licensed Clinical Social Worker (LCSW)
- Pennsylvania Licensed Professional Counselor (LPC)
- Knowledge of HIPAA compliance and ability to handle confidential information
PREFERRED SKILLS AND ABILITIES:
- Ability to work effectively in a fast-paced primary care environment.
- Excellent interpersonal skills for a multidisciplinary team collaboration.
- Proficient in brief diagnostic assessments for mental health and co-occurring disorders.
- Strong organizational skills and proficiency in Microsoft Office Suite and electronic medical records.
